ACT Rehabilitation Specialist

Southwest Network
Phoenix, AZ, US
Full-time

OVERVIEW OF POSITION

The ACT Rehabilitation Specialist plays an important role on the Assertive Community Treatment team by helping the members they serve obtain and maintain involvement in employment and / or community-based activities.

You’ll provide vocational rehabilitation services, situational assessments, and support services to our most vulnerable members and serve as the subject matter expert for the team regarding rehabilitation- and vocational-related services.

You will work with the ACT clinical team to engage, educate, communicate, and coordinate care with each member, their family, behavioral, medical and dental health care providers, community resources, and others to ensure that all services prescribed in each member’s individualized service plan are implemented.

You must be passionate about pursuing best practice outcomes for members in all areas of their lives, including continuing education, employment, community tenure, social network, independent living status, and decreased substance use.
