Mountain Park Health Center is looking for Pharmacists to join our well-established and accredited group of pharmacists who provide high quality pharmaceutical patient care and services to patients at our Baseline and Maryvale clinic sites.
As a pharmacist, you will foster the pharmacist-patient-provider relationship and motivate patient cooperation toward the delivery of care.
The Pharmacist will also review and evaluate medication orders, dispense medication, and will provide counseling and information to patients and staff about medications and their use when prescribed by physicians and other healthcare practitioners.
Essential Functions
- Maintains compliance with federal, state, and local laws and regulations as well as the organizations policy and procedures.
- Provides patient-centered care, including counseling on prescription and non-prescription pharmaceuticals, and provider or physician collaboration as needed.
- Reviews medication orders and evaluates their appropriateness and completeness with respect to patient data including age, allergies, height, weight, diagnosis, and other pertinent patient information.
- Completes prescription orders received from patients and providers to include data entry, data verification, drug utilization review (i.
e dosing and indication verification, therapeutic duplications and interactions, allergy review, and other safety verifications), filling, labeling, final product verification, and staging for delivery.
- Utilizes clinical pharmacy skills, including checking for appropriate dosage, contraindication, and duplicate therapy, in assessing and formulating recommendations to providers ensuring quality and economical drug therapy.
- Maintains accurate and up-to-date records of patient medications in pharmacy software and electronic health record.
- Ensures compliance and completeness of reporting with adverse drug reactions and medication
- Monitors and assesses 340B guidance and / or rule changes, including, but not limited to, HRSA / OPA rules and Medicaid changes to ensures that the 340B pharmacy program is continuously compliant with 340B federal regulations.
- Works at least a full 8-hour shift once every 3 months.
- Completes mandatory trainings by required deadlines.
- Works on-call schedule as needed.
- Travels to multiple locations as assigned which may include a location change during shift.
- Maintains regular and predictable attendance.
- Performs other duties as assigned.
Position Qualifications
Minimum Qualifications :
- Arizona Pharmacist License.
- Immunization certified
- Valid CPR Card
