Our company is a fast-growing part of a renowned leader in hospice and palliative care. We are seeking a compassionate and experienced Advanced Illness Management Palliative Care Nurse to provide high-quality end-of-life care to patients in their homes.
This role involves conducting in-person and telephonic assessments, educating patients and families, coordinating care with the interdisciplinary team, and documenting timely clinical records.
The ideal candidate will have at least 2 years of prior skilled nursing experience and hold a current RN license.
