As a Physical Therapist, you will be responsible for evaluating and treating residents with various physical conditions.
This includes restoring muscle strength, coordination, body alignment, relieving pain, and improving movement and function.
You will work closely with a multidisciplinary team to provide comprehensive support and information within the realm of Physical Therapy.
Responsibilities
1. Review resident information to assess its impact on the treatment process.
2. Conduct thorough functional assessments, including home / vocational and equipment needs.
3. Develop and implement individualized treatment plans with achievable functional goals.
4. Adhere to evaluation, treatment, and documentation guidelines, maintaining accurate records.
5. Instruct and supervise Assistants in treatment procedures.
6. Document each treatment session in the resident record.
7. Update treatment plans as needed and provide clear discharge summaries.
8. Orient residents to program services and inform them of any potential risks.
9. Collaborate with nursing staff, residents, and families to maximize functional abilities.
10. Educate residents and their caregivers on exercise programs and therapeutic procedures for continued home treatment.
11. Assist in the referral process for additional services and maintain knowledge of community resources.
12. Attend and participate in rehabilitation team conferences and department meetings.
13. Participate in continuing education opportunities to maintain clinical competency.
14. Contribute to the quality improvement process.
15. Promote our programs and services through interactions with the community.
16. Follow regulatory and professional standards and guidelines.
17. Provide feedback on assistant performance to the Rehab Manager.
18. Adhere to facility policies, participate in quality improvement, and safety programs.
19. Perform other duties assigned to provide quality care.
20. Understand reimbursement guidelines and allocate therapy resources accordingly.
21. Demonstrate knowledge of relevant regulations, HIPAA, and third-party reimbursement guidelines.
22. Support our commitment to Compassionate Care and maintain a professional image.
23. Carry out duties independently and recognize situations requiring supervision.
24. Maintain expected productivity levels.

Qualifications
1. Bachelor / Masters of Science degree in Physical Therapy.
2. Current state license to practice.
3. Preferably have experience in a similar setting serving the same age / type of resident population.
4. Relevant certifications and demonstrated competency in specialized areas.
5. Excellent oral and written communication skills.
6. Competency in applying physical therapy principles, methods, materials, and equipment.
7. Ability to assess resident needs and develop comprehensive care plans.
8. Knowledge of accreditation standards and compliance requirements.
9. Ability to perform physical activities required for the role.
