Swan Hospice -
BASIC PURPOSE :
Assists the Medical Director in providing medical direction for the Hospice team and overseeing the quality of medical care provided to the patients thereof during his / her absence or time of high census.
Certifies and recertifies patients for the hospice program by working in conjunction with the patient’s attending physician.
Maintains a working relationship with members of the Hospice Interdisciplinary Team (IDT) in order to assure effective comprehensive palliative care to hospice patients.
Provides education, information and updates to area physicians and other health professionals on hospice, palliative care and services as a liaison between Hospice and community physicians, medical staff within the area hospitals and hospice referral contacts.
