JOB PURPOSE :
The Medical Respite Coordinator will work in collaboration with the Medical Respite Team (MRT) to provide coordination of all medical respite activities.
This position provides oversight of the medical respite unit and team, works in concert with the National Institute on Medical Respite Care (NIMRC), and provides screenings and assessments of all medical respite referrals.
ESSENTIAL JOB DUTIES
- Provides supervision and coordination support for the medical respite team, including the medical respite case manager, and technicians.
- Serves as liaison between local hospitals and Charles Drew Health Center.
- Works with case manager to conduct an individual needs assessment and care plan.
- Review and assess all referrals submitted by hospital staff.
- Determine appropriateness and eligibility for enrollment in the medical respite program.
- Maintain the medical respite outcomes tracker, ensuring all appropriate data is up to date.
- Participate in Health and Housing Coalition meetings, providing medical respite updates.
- Participate in technical assistance activities.
- Works cooperatively / proactively with healthcare workers, case managers, clinicians, housing navigators, and shelter staff to help clients reach their goals.
- Works collaboratively with shelter supervisors and case management supervisor to help staff support medical respite guests in the shelter.
- Assist case manager in developing discharge and transition plans.
- Provides excellent internal and external customer service, behaving in a manner that cultivates collaboration, respect, and teamwork.
- Implements agency policy and procedures to effectively serve Medical Respite clients.
- Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
- Communicate effectively in oral and written forms with clients, their families, and their significant others, coworkers, supervisors, other service agencies and the community remain culturally sensitive and respectful in all client and staff interactions.
- Adhere to all client confidentiality requirements & standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state, and local standards.
- Perform other duties as assigned.
JOB REQUIREMENTS :
- Strong organizational skills.
- Knowledge of issues surrounding medical respite and patient centered care, homelessness, including mental illness, domestic violence, chemical addictions, criminal behavior, and dysfunctional thinking.
- Strong interpersonal and social skills with demonstrated ability to collaborate with a variety of organizations and individuals from a wide range of professional and personal backgrounds.
- Capacity and willingness to learn new software, research methods, and work routines quickly; flexibility in responding to information and reporting requests as they arise.
- Intermediate knowledge of Microsoft Office Word, Excel, and Outlook.
- Possess knowledge of local community agencies, programs, and available client resources.
- Ability to work with diverse populations and all levels of people within the workplace.
- Strong verbal, written, and interpersonal skills.
- Ability to work independently in a fast-paced environment and be organized with extreme attention to detail.
- Continued development of resiliency skills and practice.
- Maintains an updated system of information related to available services and programs.
EDUCATION & EXPERIENCE REQUIREMENTS
- Master’s Degree in social work is required.
- Ability to communicate effectively both verbally and in written form, relaying case information to clients / families, physicians, and other healthcare professionals.
- Excellent communication and interpersonal skills.
- Strong social services background.
- Two years employment experience in health care and case coordination preferred.
- Basic working knowledge of Windows (MSWord, Excel and publisher). Good working knowledge of e-mail and Internet,
