Essential Duties and Responsibilities include the following. Other duties may be assigned. Provide case management services to children and their families in foster and adoptive care system.
Formulate permanency goals for assigned children.Formulate concurrent plans for assigned children.Assess risk of child abuse / neglect and on-going safety of children.
Assess child and family functioning, family strengths, family needs, stressors and coping mechanisms.Research and refer children and families for needed services.
Evaluate the impact of services.Testify on behalf of assigned children in court.Provide parent education.Participate in on-call rotation.
Comply with all required contractual documentation standards.Provide face-to-face meetings with assigned children and out-of-home care providers according to prescribed time frames.
Schedule, coordinate and facilitate FST meetings within prescribed time frames.Secure appropriate and least restrictive out-of-home placements for assigned children.
Arrange for medical and dental exams, medications, treatment testing or routine health services for assigned children in out-of-home placements.
Assist families in meeting the cost of care expenses. Job Type : Full-time
