Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients. They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.
The ideal candidate should be detail-oriented, patient and calm. Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.
Responsibilities
Hours of operation : 12pm - 10 : 30pm and every other weekend
Duties and Responsibilities
- Carries primary responsibility for evaluation of all patients referred for hospice services.
- Is able to articulate all components of the organization's continuum of care and refer patients and families to those as necessary.
- Assists referring physicians and / or his / her designated liaison with coordinating a plan of care / treatment for hospice appropriate patients and their families.
- Serves as the liaison between our company and physicians, hospitals, long-term care facilities, and other health care professionals.
- Actively markets, through education, the hospice concept to all health care professionals and providers, as well as the community at large.
Qualifications
A. Education / Regulatory Requirements :
- Florida licensed R.N.; B.S.N. preferred.
- 1 year medical surgical experience preferred
B. Skills :
- Preferred CHPN Certification.
- Must possess ability to assess physical and psychosocial needs of patient and family. Hospice experience highly desirable.
- Demonstrates excellent verbal and written communication skills. Bilingual Preferred. Electronic medical record software experience a plus.
- Organized, flexible, self motivated with ability to relate to all levels of the professional medical team.
- Ability to develop and apply effective customer service skills. Able to work independently with minimum supervision.
