This position coordinates the delivery of quality interdisciplinary care for the patients and families in the hospice program and acts as a resource and back-up to the nursing and aide staff to ensure continuity of care.
Assumes management in the absence of the Nursing Director for Homecare Services.
Essential Duties
- Provides supervision of PRN RNs, LPNs, and nursing assistants to ensure that care provided is appropriate, delivered according to the approved plan of care and is fully compliant with all applicable rules, regulations and standards of practice.
- Responsible for related personnel functions for said employees including hiring, orientation, performance evaluations, counseling, payroll, professional development and employee problem resolution.
- Assigns patient’s nursing assistants and assist with scheduling based on the patient’s identified needs, staff competencies, licensure, and situational / procedural complexities.
Assigns RN Case Managers in the absence of the Director of Homecare Services.
Facilitates bi-weekly interdisciplinary team meetings and ensures that appropriate documentation of the team meeting is maintained.
Helps the teams establish appropriate patient care priorities and plans of action and monitors to ensure compliance.
- Responsible for reviewing patient medications to ensure that drugs and biologicals meet each patient’s needs. Consults with physicians and pharmacists, as necessary, to ensure patient’s needs are met.
- Audits nursing documentation to assure that it is complete, current and that the care provided is appropriate for the patient’s circumstances and is provided according to the physician’s orders and organizational policy;
audits completion of recertification orders in a timely manner.
- Triages patient, family and medical community phone calls in the absence of the primary care team members.
- Makes home visits as necessary to assist staff nurses in following plan of care.
- Communicates with IPU Director and staff to promote effective care quality operations.
- Ability to lift, turn and transfer patients; minimum lifting requirement 40 pounds
- Participates in administrative on-call schedule.
