Responsibilities
Provide expert pharmaceutical care to patients, ensuring optimal therapeutic outcome
Collaborate with healthcare professionals to develop and implement individualized treatment plan
Conduct comprehensive medication reviews and monitor patient responses to therapy.
Educate patients and healthcare staff on medication management and best practices.
Participate in clinical research and contribute to the advancement of pharmaceutical knowledge.
Maintain accurate and detailed patient records in compliance with regulatory standards.
Stay current with advancements in clinical pharmacy through continuous professional development.
Qualifications
Holds a Doctor of Pharmacy ( degree from an accredited institution
Possesses a valid and active pharmacist license in the relevant jurisdiction.
Completed a PGY-1 residency in pharmacy practice and a PGY-2 residency in a clinical pharmacy specialty.
Demonstrates extensive experience in direct patient care and medication therapy management.
Exhibits strong knowledge of pharmacotherapy, clinical guidelines, and evidence-based practices.
Proficient in the use of electronic health records and pharmacy information systems.
Maintains excellent communication and interdisciplinary collaboration skills.
