Job Overview
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring they receive the necessary support for their health and well-being.
This role requires strong clinical skills, effective communication, and a commitment to patient-centered care.
Responsibilities
- Conduct comprehensive assessments of patients’ health status and develop individualized care plans.
- Administer medications and treatments as prescribed while monitoring for side effects and effectiveness.
- Provide education to patients and their families regarding care plans, medication management, and health maintenance.
- Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
- Maintain accurate documentation in compliance with HIPAA regulations and Medicare guidelines.
- Manage patient records and ensure all documentation is completed timely and accurately.
- Support patients with activities of daily living (ADLs) as needed, including mobility assistance and personal hygiene.
- Participate in quality improvement initiatives to enhance patient care services.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in home health, hospice care, or nursing administration preferred.
- Strong knowledge of anatomy, nursing management principles, and care plan development.
- Familiarity with Medicare regulations and compliance standards is highly desirable.
- Excellent communication skills with the ability to build rapport with patients and families.
- Ability to work independently while managing multiple priorities effectively.
- Compassionate nature with a strong commitment to providing high-quality patient care.
