Position Description
The Case Manager is responsible for completing clients' reports and connecting the client to needed resources.
Qualifications
- A bachelor's degree, preferably in social work, psychology, or an equivalent discipline.
- A minimum of one year experience in a residential setting, preferably with troubled latency and adolescent clients.
- Pass a criminal background check (FBI, DOJ, and Child Abuse), TB test, physical exam, and drug test.
- Have a valid driver's license and meet insurance requirements.
- Current CPR and First Aid certificates.
- The ability to author emails, letters, reports, and other documents professionally.
- Proficient with Microsoft Office Suite or related software.
- Organizational skills that include managing time efficiently and workload effectively.
- Exceptional communication skills, both oral and written.
- Strong leadership and management abilities to effectively oversee staff and operations.
- The ability to work collaboratively within a team environment and inspire a positive and inclusive work culture.
- The ability to work effectively under pressure, managing multiple tasks and responsibilities simultaneously.
- The ability to work independently, be a self-starter, and be energetic.
- Must be able to work a flexible schedule that may include extended hours, evenings, weekends, and holidays.
- The personnel file shall contain documentation of the required education and experience, an age affidavit, and a confidential form.
Examples of Duties and Responsibilities
- Collaborates with the client, staff, CFT, and other supportive entities to meet the client's needs.
- Prepare the Quarterly, Discharge, and Needs and Services Plan reports as required, ensuring that all required signatures are included.
- Responsible for ensuring that all reports are turned in on time and approved by the HOS.
- I track report due dates and coordinate with the administration to ensure that information required to complete reports is received in a timely manner.
- Meet regularly with the clients and the treatment team to establish goals.
- Assists with linking clients to needed services, including scheduling external appointments.
- Assist the HOS with quality assurance duties pertaining to mental health, including but not limited to audit preparation, chart evaluations, and file maintenance.
- Attend treatment team, case conferences, and CFT meetings.
- Adheres to the agency's safety protocols.
- Organize and facilitate resident council.
- Transport clients to ILS activities.
- Other duties as assigned.
