AbbeHealth Services - is a regional nonprofit organization dedicated to providing mental health and aging services that help individuals achieve their optimum levels of independence and success.
Performs professional social work in the general social service field of mental health; does related work as required. Report to the Administrator or designee.
Assists individuals and their families with personal and environmental difficulties, which predispose illness and interfere with obtaining maximum benefits from medical care.
Works in close collaboration with physicians and other health care personnel in individual evaluation and treatment to further understanding of significant social and emotional factors underlying individual’s health problem.
Helps individual and family through individual or group conferences to understand, accept and follow medical recommendations.
Provides service planned to restore individual to optimum social and health adjustment within individual’s capability. Utilizes community resources to assist individual to resume life in a community or learn to live within limits of disability.

Responsibilities
- Participate in program team meetings.
- Assist with assessment of referrals for the subacute facility and facilitation of tours and admissions.
- Assist with assessment of hospitalized residents and coordination of their return to the subacute facility.
- Ensure that all required funding and paperwork is in place for admissions prior to the admission date.
- Participate in collaborative meetings with other agencies as requested.
- Provide reports on admissions and discharges to the leadership as requested.
- Stay current on relevant rules and regulations and make recommendations for changes in policies and procedures to the Manager and Administrator.
- Review and revise the social work manual as needed.
- Maintain a social work caseload of subacute residents.
- Complete all documentation as required, such as progress notes, service plans, quarterly reports and incident reports.
- Prepares individual history, services, plans and reports.
- Participates in the planning for improving health services by interpreting social factors pertinent to the development of program.
- Assembles case history files as appropriate.
- Provides counseling in family problems and socialization.
- Refers individuals to other sources of assistance.
- Conducts interview to obtain necessary statistical information for case summaries and histories.
- Consults with other community agencies, people or institutions also involved with this individual.
- Participates in discharge planning for individuals.
- Work as a team with other employees to create a welcoming and engaging environment.
- Attend and participate in continuing education programs to maintain current knowledge and skills.
- Assist with marketing subacute facility to referral sources and the community.
- Participates in the planning, implementing and evaluation of programming effectiveness in subacute services
- Provides other leadership functions as assigned by the Crisis Manager.
- Perform all other duties assigned.
Qualifications
- Bachelor’s degree in a Human Services related Degree
- Requires a minimum of 1 years work experience with an emphasis in human services / mental health / intellectual disability, and / or gerontology.
- Valid Iowa Driver’s License
