About the Role : Job Summary
Job Summary
Responsible for development and coordination of patient assignments daily. Works closely with Therapy Supervisor to coordinate staff usage, competence and delivery of services.
Provides and documents counseling services including group therapy, individual therapy, social assessments and appropriate discharge planning.
Provides service for a full range of patients, including adolescents and geriatrics.
Minimum Job Qualifications
Licensure or other certifications : Georgia licensure as Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT).
Educational Requirements : Masters degree in Social Work, Psychology, Counseling or other related field that meets the requirements for licensure in the state of Georgia.
Minimum Experience : Minimum of four years of experience in Behavioral Health field.

Essential Tasks and Responsibilities
Works with the Clinical Coordinator and Nurse Manager to develop patient schedules for inpatient, DPH and IOP levels of care.
Evaluates the ongoing needs of the patient population and community to help management determine long term focus.
Meets routinely with Clinical Coordinator, Nurse Manager, House Coordinators, Medical Director and Manager of Clinical Services to seek program feedback for further development of program.
Provides community education to employee groups, other Mental Health Agencies and individuals to further build Laurelwood’s reputation in the community.
Serves as a liaison for staff and the program to the Medical Director.
Displays a team approach to problem solving, utilizing those on their team necessary for solving daily problems that may arise.
Meets formally and informally with aforementioned staff to evaluate staff competency.
Evaluates educational needs of staff on these units, to assure competence and quality of program.
Assists Clinical Coordinator with ongoing staff evaluation, identifying areas needing individual growth or development.
Participates with the Clinical Coordinator in budget review to helps maintain the program within budgetary limits as needed.
Participates in Laurelwood supervisor / manager meeting, as needed, to learn the budget process and help administration identify areas of further savings.
Assist Clinical Coordinator with FTE utilization and management.
Serves as primary liaison between referral sources of patients and the Laurelwood program to ensure continuity of care and customer satisfaction.
Coordinates the treatment team and planning process with other multidisciplinary team members to ensure that planned treatment occurs, and assumes leadership role in documentation process.
Completes a psychosocial assessment and master treatment plan on all assigned patients on day 1 of treatment.
Documents all significant contact with patient’s family in the patient’s chart on the day they occur.
Provides family therapy as requested by the treatment team, trouble-shoots high profile, labor intensive cases.
Demonstrates clinical competency in the provision of group sessions to ensure effective treatment and a safe therapeutic milieu.
Coordinates discharge planning for assigned patients to ensure appropriate continuity of care upon discharge.
Works as a team player with all staff to ensure the safe, productive, and effective operation of Laurelwood.
