Social Worker - Care Navigation
Job Summary : Provides psychosocial assessments, plans, conducts, and coordinates care planning for complex cases and participates in multidisciplinary team processes.
Victims of domestic violence, child and adult neglect and abuse, adoptions and other DSS issues shall be the Social Worker responsibility.
The Social Worker shall provide education and resource identification while providing quality service with cost reduction in mind.
Must be knowledgeable with State and Federal laws as they pertain to adult and child abuse reporting, adoptions, and domestic violence.
The Social Worker will function as a resource consultant for providers, clinics, and navigators with individualized patient care planning.
Compliant with State, Federal, Medical Group Practice guidelines. Utilizes technology and reports such as EHR, disease registries, PCP platforms, Managed Care, MSSP, and Medicaid reports to perform job role.
Qualifications : Bachelor’s degree in social work or related field from an accredited college or university with a minimum of 5 years’ experience in social work required;
certification preferred, LCSW or MSW preferred. Extensive community and mental health experience preferred. Thorough knowledge in principals and methods of social work, including guardianship, adoptions, neglect and abuse, undocumented citizens, and placement practice.
Advanced skills in negotiation with State, Federal and community facilities and agencies. Knowledge of State and Federal laws as they relate to social work reporting practices.
Exemplary verbal and written communication, creative thinking, and cost savings practice. Proficient with Microsoft Programs.
