PRIMARY FUNCTION
The Community Housing Placement Case Manager is responsible for providing ongoing support and resources to clients in the Emergency Housing Assistance Program.
This full-time position focuses on ensuring housing stability and overall well-being for individuals in the LGBTQ+ community who are affected by HIV / AIDS and have experienced or are at risk of homelessness.
The Community Housing Placement Case Manager coordinates services, tracks care, and connects clients to social assistance and support networks.
DUTIES AND RESPONSIBILITIES
- Provide case management services to clients in the program, including regular check-ins, goal setting, and monitoring progress.
- Coordinate necessary services and resources for clients, such as healthcare, vocational support, government benefits, and support groups.
- Develop individualized care plans for clients, focusing on housing stability, overall health, and increased viral suppression.
- Connect clients to community resources and support networks to enhance their well-being and social integration.
- Maintain accurate and up-to-date client records, documenting all interactions, assessments, and interventions.
- Collaborate with other team members and external stakeholders to ensure a holistic and comprehensive approach to client care.
- Provide crisis intervention and support as needed, advocating for clients’ needs and rights.
QUALIFICATIONS
- Bachelor’s degree in social work, psychology, or a related field.
- Experience in case management, preferably in the field of HIV / AIDS or homelessness.
- Knowledge of LGBTQ+ issues and familiarity with gender-affirming care practices.
- Understanding of housing instability and its impact on overall health and well-being.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients.
- Strong organizational and time management abilities, with the capacity to handle multiple tasks and prioritize accordingly.
- Proficiency in documentation and record-keeping.
- Knowledge of local community resources and service providers.

WORK HOURS :
Regular office hours are 8 : 30 a.m. to 5 : 00 p.m. Monday through Friday.
SALARY RANGE :
$50,000.00 $53,000.00 per year
