LionHeart Eldercare & Consulting is looking for an experienced senior care professional to join our fast-growing team of Care Managers serving Northern Virginia older adults and their families.
Do you love working with older adults and thrive on helping advocate for their needs and desires? You can play a critical role in supporting your clients’ purpose and joy while also supporting their family and care partners.
Responsibilities include :
- Meet with clients to assess and coordinate their care needs
- Advocate on the client's behalf with facilities, physicians and family members
- Attend appointments, schedule meetings and manage care plans for clients
- Coordinate and collaborate with trusted community partners
Qualifications :
- Bachelor's degree and 3 to 5 years of relevant experience. Experience in geriatric care management, health care, medical social work or case management preferred
- Only candidates who CURRENTLY RESIDE in eastern Fairfax, Arlington or Alexandria counties will be considered.
- Must have a computer and ability to maintain electronic records.
- Use of personal car is required. Appropriate licensure and car insurance required.
