We are seeking an experienced Nephrology Nurse Practitioner to join our Risk Adjustment team, specializing in end-stage kidney disease (ESKD) care.
As a Nephrology Nurse Practitioner, you will play a crucial role in case management for patients with kidney disease, providing comprehensive assessments, education, and care.
Additionally, the NP will collaborate with nephrologists and the care team to complete rounding in the dialysis centers alongside the care team monthly.
The main role involves rounding to meet and treat patients and conducting annual wellness visits for the value-based care risk adjustment program, which will take place at the Center or Practice Office.
The job may include telehealth (20% of the time) and home visits (10-20% of the time as needed).
Schedule : M-F 5x8's or 4x10's - flexible schedule
Must Haves :
- Current specialty certifications and license to practice as an Advanced Practitioner in the state
- Enrolled in CMS (Medicare) of the state
- Firm understanding of the protocols, policies, and operations of a physician’s practice
- Minimum of 2 years of NP experience
- Experience working in Nephrology and / or internal medicine specialties (e.g., cardiology, endocrinology)
- Excellent written and verbal communication skills
- Positive and charismatic attitude
- Comfortable traveling between facilities and working independently
- Familiarity with Risk Adjustment in Value-Based Care (highly preferred)
Responsibilities are listed below :
- CKD Clinical Care by offering clinical care and kidney disease education.
- Provide specialized care to patients with kidney diseases, manages dialysis treatments, and collaborates with physicians
- Risk Adjustment Documentation
- Conduct comprehensive assessments with a focus on risk adjustment documentation.
- Holistic Patient Care Management
- Manage end-stage kidney disease patients holistically, addressing activation, depression, hospitalization risk, transitions of care, and co-morbid medical issues.
- Review patient charts for chronic kidney disease (CKD) and make suggestions as needed
- Assist in referrals to IDT teams (referring case management or social worker follow-ups)
- Travel between multiple dialysis facilities and work independently
- Completing rounding to meet and treat patients
