GENERAL SUMMARY

Clinical Management Scope / Outcome Responsibility :

Reporting to the Director of Social Work and Clinical Services, the Social Work Manager is responsible for the daily operation of clinical social work activities including patient / family care issues, social work staffing / assignments, consultation, education, program development, training and mentoring for inpatient and / or ambulatory areas.

Supervision / Operations :

Creates an environment that empowers staff to provide high quality patient and family centered care. There is accountability for ensuring efficient and professional social work services for patients and families that are designed to promote and enhance their physical and psychosocial functioning with attention to the social and emotional impact of illness and disability.

Monitors efficient utilization of healthcare resources and LOS issues within the service lines. Included is a responsibility for establishing and maintaining productive working relationships with social work leadership, care coordination leadership, medical and nursing staff, ancillary departments, the healthcare team, community agencies, governmental agencies, health system leaders, and resource providers.

As a member of the Social Work and Care Coordination administrative team, provides leadership and consultation to Care Coordination staff, medical and nursing staff and hospital administration.

Participates in departmental operations, including administrative support systems, budgets, quality improvement and assessment to optimize care resource utilization along the full continuum of care, thereby facilitating achievement of optimal quality and fiscal outcomes.

Establishes effective working relationships with external representatives from the community including Protective Services, DPH, DMR, DMH, Medicaid, home health, community, and other state and federal agencies.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Leadership / Professionalism :

As a member of the Department’s Social Work Leadership Team, participates in evaluation and planning for the Department’s overall clinical practice, policies and procedures, quality improvement and strategic direction inside the hospital and in the community.

Joins with other members of the department leadership to provide the organizational direction necessary to effect change, reduce cost and promote excellence.

Represents assigned teams and the department on hospital and departmental committees and task forces. Collaborates with staff and leadership in multiple departments within the hospitals as assigned.

Clinical :

  • Plan, develop, implement, and assist in managing a program of clinical social work activities for multiple clinical service inpatient service lines.
  • Responsible for the quality and quantity of clinical services for assigned teams.
  • Provides oversight of case assignments, productivity standards and time management for social work. May initiate studies of assigned team activities including productivity, staffing and efficient use of resources.
  • In partnership with the care coordination leadership, develops and monitors roles and expectation of the social workers, care coordination nurses and resources specialists to support the integrity of their respective practice or disciplines.

Supervision / Operations :

  • Provide supervision to staff social workers.
  • Creates an environment that encourages individuals / teams to excel in clinical practice, teaching and research.
  • Screens applicants for employment, leads team interviews, recommends candidates for hires to Director.
  • Coordinate and provide adequate staffing and coverage for clinical social work needs for multiple clinical service lines.

Ensure all units are properly covered on a day-to-day basis.

  • Performs annual performance evaluations. Processes requisitions, Kronos and PeopleSoft transactions.
  • Provides leadership to assigned teams. Promotes positive team relationships by encouraging open dialogue and facilitating a team approach to service delivery.
  • Evaluates and collaborates with other social work projects and / or interventions that address effectiveness on patient / family outcomes.

Identifies trends or changes in patient responses to interventions and uses data to evaluate and improve systems.

  • Demonstrates understanding of and applies good financial management practices in carrying out assigned responsibilities.
  • Adjusts rapidly to new situations and helps staff to adapt to changing situations or unexpected obstacles.
  • Maintains focus, intensity, and persistence, even in adverse, evolving, or ambiguous situations.

Leadership / Professionalism :

Functions as a member of the Department’s Leadership Team to develop and implement clinical standards, policies / procedures, quality improvement activities and to set strategic vision for the department.

Attends service line LOS meetings. Prepares social work staff for attendance accordingly.

  • Keeps informed of evolving clinical theory, practice and issues affecting social work in health care and proactively recommends changes.
  • Executes special assignments / responsibilities from the Director of Social Work.
  • Establishes, maintains, and promotes effective working relationships with leaders and colleagues.
  • Maintains professional Social Work licensure in Massachusetts.
  • Pursues continuing education opportunities in social work and management.
  • Provides advanced clinical services as needed. Participates in administrator on-call. Other duties as assigned by the Director.

Qualifications

QUALIFICATIONS :

EDUCATION : Master’s Degree in Social Work from an accredited program required.

LINCENSURE : Current Social Work licensure in Massachusetts required.

EXPERIENCE :

  • Previous clinical social work experience in a hospital setting required.
  • Previous management experience preferred.

LANGUAGE : Bilingual (English / Spanish) preferred.

WORKING CONDITIONS

The Department of Care Coordination will operate seven days a week / 24 hours a day. Hours and work schedule will be flexible to meet needs of patients, families, hospital / system and staff.

Must be prepared to come in to work or stay at work during a hospital emergency.

SUPERVISORY RESPONSIBILITY

  • Manages social work staff, social work interns, social work on-call and a resource specialist at BWH, consultation to care coordination nurses other interdisciplinary staff and departments.
  • Deals appropriately with employee issues and ensures that staff is treated in a fair and equitable manner.
  • Creates a work environment that encourages creative thinking and innovation. Inspires, motivates and guides others toward goal accomplishment.

FISCAL RESPONSIBILITY

Responsible for fiscal operations of social work and social work billing. As a member of Care Coordination management, participates in departmental budget reviews to assure operational efficiency of Care Coordination.

HOSPITAL WIDE RESPONSIBILITIES

  • Works within legal, regulatory, accreditation and ethical practice standards relevant to the position and as established by BWH / Partners;
  • follows safe practices required for the position; complies with appropriate BWH and Partners policies and procedures; fulfills any training required by BWH and / or Partners, as appropriate;

brings potential matters of non-compliance to the attention of the supervisor or other appropriate hospital staff.
