Responsibilities
- Provide Physical Therapist services including assessment, evaluation, and teaching activities
- Update care plans as needed and submit visit notes as requested
- Perform OASIS time point assessments and discharge planning
- Attend staff meetings, team conferences and educational in-services
- Use the 4Ms and provide Age-Friendly Care
Qualifications
- Current State Physical Therapy License
- Good driving record, proof of insurance, and reliable transportation
- One year home care experience preferred
- Ability to make sound clinical judgment and assess patient needs
- Proficiency in Microsoft Office

Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.
