PURPOSE ANDSCOPE :

The Inpatient RNprovides direct patient care in accordance to company policies andprocedures which includes FMCNA compliance programs and contractedhospitals' policies and procedures.

Areas of practice are diverse,including, but not limited to, hemodialysis, peritoneal dialysis,continuous renal replacement therapies, apheresis, andultrafiltration.

The Inpatient RN will partner with the hospital toprovide quality care to the patient and deliver care that providesa positive patient experience.

PRINCIPAL DUTIES ANDRESPONSIBILITIES :

  • Underlimited supervision, applies considerable knowledge to perform abroad range of tasks which include but are not limited to directpatient care in multiple modalities not limited to hemodialysis,peritoneal dialysis, ultrafiltration, continuous renal replacementtherapy, and Apheresis.
  • Provides supervisionand direction of Patient Care Technicians and clerical staff;provides direction and education to ICU staff forCRRT.
  • Responsible for direct patient care ofassigned patients in the Inpatient Renal Replacement and ApheresisServices setting and providing the appropriate treatment techniquefor assigned patients as prescribed in all modality procedures, asappropriately defined - hemodialysis, peritoneal, continuous renalreplacement therapies, ultrafiltration, andApheresis.
  • Assesses patients' responses totreatment therapy making appropriate adjustments and modificationsto the treatment plan as indicated by the appropriatelycredentialed physician.

Communicate problems or concerns to theInpatient Program Manager, appropriately credentialed physician,patient hospital primary nurse and others as indicated.

  • Delivers safe, effective care in a timely anefficient manner.
  • Gives organized, conciseshift reports for patients on assigned shifts. Apply high levelthinking skills to shift patient schedules to meet needs of morecritical patients due to labs, MD order, or patientassessment.
  • Responsible for the implementation,administration, monitoring, and documentation of patient's responseto prescribed intradialytic transfusions, including appropriatenotification of adverse reactions to physician and appropriateblood supplier.
  • Collaborates and communicateswith physicians and other members of the healthcare team tointerpret, adjust, and coordinate daily patient care plan to ensurecontinuity of care.
  • Initiates and communicatesto FMS facility nursing staff and appropriate hospital personnel asneeded.
  • Accurately documents all treatmentinformation in the individual patient record (ACES) and thehospital record, as needed.
  • Cleans anddisinfects dialysis machine surface, chair, equipment, andsurrounding areas between treatments according to inpatient renalservices policies and procedures.
  • Conducts alltasks necessary for preparation for dialysis treatment anddocuments where appropriate.
  • Performs allrequired pre-treatment dialysis machine alarmtesting.
  • If applicable, initiates SolutionDelivery System.
  • Operates all related equipmentappropriately and safely and provides minor trouble shooting whennecessary; efficiently utilizes supplies to preventwaste.
  • Understands, conducts, and documentsappropriately Dialysis / Apheresis machine safety tests / alarm tests,equipment calibration, dialysate testing, machine safety tests,functional testing, and internal and external disinfection on allwater & dialysis machines, and complies with thedocumentation / notification standards per FMSpolicies.
  • Completes and documents ongoingparticipation in QAI activities.
  • Participatesin staff meetings as scheduled.
  • Participates instaff training and orientation asassigned.
  • Acts as a subject matter expert tohospital staff for dialysis related patient / equipmentcomplications.
  • Responsible for reporting allnew or unusual incidents, information, complaints, or problems tothe inpatient manager and other parties per the Adverse EventReport requirements.

May be assigned to in-center patient care onan as needed basis

  • Review and comply with theCode of Business Conduct and all applicable company policies andprocedures, local, state and federal laws andregulations.
  • Assist with various projects asassigned by direct supervisor.
  • Other duties asassigned.

Direct patient care,interaction with patients, outpatient clinicians, hospital staff,and physicians. The position requires travel between assignedhospitals and facilities;

as well as travel to regionalmeetings / educational events.

  • Position requireson call rotation including night, day, weekend, andholidays.
  • There is potential for exposure topotentially infectious material and contagious diseases / material.
  • Position provides direct patient care whichregularly involves heavy lifting, moving of patient and / orequipment >

200lbs (with assistance); and may lift chemical andwater solutions of up to 30lbs up as high as 5 feet.

SUPERVISION :

Provides supervision anddirection of Patient Care Technicians (Certified ClinicalHemodialysis Technicians (CCHT and clerical staff;

providesdirection and education to ICU staff forCRRT.

EDUCATION :

  • Currentappropriate state licensure.
  • Maintains acquiredknowledge and remains current in advancements in nephrology, inparticular, relative to inpatient care (hospitalized patientsrequiring varied treatment modalities, not limited to RenalReplacement, Apheresis, and Ultrafiltrationservices).
  • Must meet the practice requirementsin the state in which employed.
  • Certificationin Nephrology Nursing, Apheresis Nursing, or both is highlyencouraged.

EXPERIENCE ANDREQUIRED SKILLS :

  • One year RN experience required andrequired nursing skills.
  • Minimum 6 monthsnephrology experience required before working independently with noother Dialysis RNs to provide dialysis and ultrafiltrationservices.
  • Minimum 6 months Nephrologyexperience required before working independently with no otherApheresis RNs to provide Apheresisservices.
  • Demonstrated strong assessment skillsrelated to the critically ill patient.
  • ICUexperience preferred but notrequired.
  • Successful completion of trainingcourse in the theory and practice of care of the ESRD patient -Progressive Renal Education Preparation.
  • BLSCPR Recognition.
  • ACLS CPR Recognitionpreferred, but not required.
  • Demonstratesorganizational and time management skills, and a competency towardcompleting continuing and ongoing Renal / Nephrologyeducation.
  • Displays leadership skills andability to direct and supervise others (such as PCTs, CCHTs,clerical staff).
  • Successful completion ofNurses Technical Training.
