Employment Type : Part timeShift : Description : Coverage Area : Farmington Hills, Bloomfield & PontiacPosition Purpose : Our Occupational Therapists provides professional occupational therapy services to the agency’s clients in accordance with the plan of care as prescribed by the physician and in compliance with the state’s professional practice act for occupational therapy and any applicable licensure / certification requirements.

Supervision ExercisedProvides functional supervision to HHAs.What You Will Do : Effectively and efficiently prioritizes visit content and plans visits based on client capabilities, condition, preferences (when possible), physician orders and available resources.

Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Organizational Integrity Program, Code of Ethics, as well as agency policies and procedures and adheres in a manner that reflects honest, ethical and professional behavior.

Accurately assesses clients at admission, at required time points, and as needed, utilizing appropriate standardized documentation.

Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.

Develops appropriate assessment-based plan of treatment with physician and client. Plan includes attainable episode goals and agency services needed to improve and / or stabilize client’s medical condition and promote independence.

Participates in timely revision of the plan of treatment as appropriate.Makes appropriate referrals for evaluation / care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.

Utilizes interview, observation, and evaluation in assessing clients and applies professional judgment, consistent with practice standards, in formulating interventions and making recommendations to the physician and client.

Evaluates the home environment and makes recommendations for the establishment of effective household management.Maintains communication regarding the client’s care / needs with the physician, therapy supervisor, other disciplines, and family, as appropriate.

Coordinates client care with other health team members : updating other disciplines regarding changes in the plan of care and the client’s condition when indicated;

and coordinating referrals, recertification, and discharge information.Notifies physician of changes in the client’s condition and / or need for additional orders, obtaining and documenting verbal orders per policy.

Eligible for the National Registration Examination of the National Board for Certification in Occupational Therapy (NBCOT).

Registration by the NBCOT and registration to practice as an Occupational Therapist in state of practice if state has registration practices.

Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with the agency’s clients and their families.

Must accept responsibility for maintaining clinical practice skills, learning and adhering to agency’s policies and procedures on an on-going basis.

Must have a minimum of one (1) year experience or have a preceptor for a minimum of 6-8 weeks.Home healthcare experience preferred.

Must have current Driver’s license and reliable transportation to and from work site.Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics and adhere to the Compliance Program.
