Job Description :
The Qualified Mental Health is responsible for the provision of services to individuals enrolled in Mental Health Skill Building, Community Stabilization, and / or Mobile Crisis.
- Assist in the initial development and ongoing revision of Individual Service Plan
- Implement Individual Service Plan
- Perform case management functions of linking, arranging for services, and referrals
- Maintain continuous contact with the identified individual and other key stakeholders’ of the individual (with signed authorization to release information) in planning and service delivery.
These stakeholders might include but are not limited to social workers, psychiatrists, court officials, police, and other human service professionals.
- Attend Supervision and meetings as required
- Complete other duties as assigned
Qualifications :
- Must have a Bachelor or higher degree in Human Service Related field
- Must be registered with the Virginia Board of Counseling as a Qualified Mental Health Professional - Adult (QMHP-A) or Qualified Mental Health Professional - Trainee (QMHP-T)
- Must be a Qualified Mental Health Paraprofessional for Mental Health Skill Building only
- Must successfully complete a criminal background and central registry check
- Valid Driver’s License
- Must have current auto insurance
- Must have current TB screening
- Must be currently certified in CPR and First Aid
