Job Summary :
As an External Case Manager with C omplete Care at Berkshire , you will play a key role in ensuring smooth transitions for patients from hospitals to our post-acute care facility.
Your dedication and compassion will profoundly impact the lives of our residents, ensuring they receive the exceptional care they deserve.

- Collaborate with an experienced and successful center leadership team.
Responsibilities & Qualifications of an External Case Manager :
- Assess patients being discharged from hospitals to determine their care needs in the post-acute setting.
- Collaborate with hospital staff, patients, and families to develop comprehensive transition plans.
- Facilitate effective communication between hospital and facility staff, patients, and families.
- Provide patient and family education.
- Identify opportunities for increased market share from assigned hospital accounts.
- Maintain excellent organizational skills.
- Ensure timely communication and acceptance of hospital referrals.
Qualifications :
- Bachelor’s degree in healthcare administration, business, or a related field (preferred).
- 3-5 years of experience as an Admissions Director, Hospital Liaison in a skilled nursing / LTACH, or acute rehab hospital.
- Strong communication and interpersonal skills.
- Knowledge of healthcare regulations and standards.
