What you’ll do

You will focus on the emotional and psycho-social needs of the residents while maintaining a relationship with families as well.

The residents will rely on you and your knowledge of the healthcare system and will look to you for guidance when it comes to navigating their care options and overcoming any barriers they encounter.

What it takes

  • Bachelor’s degree in social work required and a current unencumbered license to practice as a Licensed Social Worker in the state of Ohio
  • Two years experience in a healthcare and / or human services environment required
  • Experience in patient advocacy required

Ohio Living Home Health and Hospice in Greater Toledo is a premier provider in Defiance, Erie, Fulton, Hancock, Henry, Lucas, Ottawa, Sandusky, Seneca and Wood counties offering hospice and palliative care, private-duty home care, long-term nursing care, physical / speech / occupational therapies, home health aides, social work services, medication administration, wound care management, IV therapy and more.
