United Hospice, Inc. is a community-based, not-for-profit that provides care and support to the communities of Rockland and Orange Counties in New York.
United Hospice envisions a community in which all individuals and their loved ones facing serious illness retain their dignity and hope while receiving the best care and support that hospice has to offer.
We are currently seeking a Physical Therapist for Orange County to work on a per diem basis and join our team of dedicated hospice professionals.
Rate of pay for this position is up to $60.00 / hr. and is commensurate with applicable experience.
Job Description Summary :
The Physical Therapist (PT) evaluates patients, develops effective treatment plans, and collaborates with physician to establish patient goals.
Responsibilities of Physical Therapist will include, though not be limited to :
- In collaboration with the physician, the PT helps develop a physical therapy plan based on patient evaluation and established goals.
- Initial assessment and evaluation of patient’s physical ability with follow up visits per MD order.
- Identify problems and develop therapy care plan according to patients’ needs with adjustments to care plan as appropriate, reviews care plan with the care team.
- Treats patients to relieve pain, develop or restore function, and maintain maximum performance.
- Directs and assists patients in active and passive range of motion exercises, muscle re-education, and functional training activities of daily living including transfer activities.
- Observes, records and reports to the patient, family, physician the patient’s reaction to treatment and any changes in the patient’s condition.
- Educate caregivers on treatment plans, evaluating their ability to assist patients.
- Participates in team meeting as appropriate.
- Participates in QA / PI program and hospice sponsored in-service training.
- Accepts other responsibilities as assigned.
Qualifications :
- New York State license in Physical Therapy is required
- At least one year experience as a physical therapist
- Excellent oral and written communication skills
- Ability to problem-solve effectively
- Ability to relate to a diverse population and workforce in a personable and effective manner
- Commitment to the hospice philosophy of care
- Must be proficient in Microsoft Office, EMRs and able to learn new software
- Must have access to a car, valid driver's license, and current insurance
