Job Description
Responsibilities :
- Assist with development and implementation of support programs that directly support the strategic plan and vision of Northwestern Medicine.
- Manage, assess and report on the quality of participants / patient care delivered and coordinate patient care services with patients, staff, and leadership.
- Coordinates daily activities of the case management team members to ensure adequate coverage for hospitalized patients.
- Assists unit based staffing assignments to achieve high quality and cost effective care.
- Assists schedule management and related processes (maintaining master schedules, entering schedule changes, PTO requests, etc.).
- Collaborates with manager to assist with interviewing of new team members, posting of new requisitions and coordination of new hire on boarding (i.
e. paperwork, IT access, lockers, etc.).
- Collaborates with manager and nurse clinician in development of new hire orientation calendar and manual.
- Contributes to and assists manager with performance evaluations.
- Assists with & reinforces iCARE coaching, hourly rounding & all other Patient Experience initiatives.
- Serves as a resource to staff and manager.
- Participate in development and execution of process and quality improvement initiatives and track outcomes and productivity.
- Contributes to development of departmental goals and report on progress with management.
- Assists manager in overseeing any special fund budgets.
- Attend monthly leadership meetings and provide progress reports as requested.
- Participate in quality meetings as needed.
- Performs other duties as assigned.

Qualifications
Required :
- Masters degree in social work or Bachelors of Science in Nursing.
- Licensed clinical social worker in the state of Illinois (LSCW) or Registered Nursing license issued by the State of Illinois.
- 5 to 7 years work experience in hospital setting or related setting.
- Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
