The Patient Intake Coordinator -
Student is a temporary position designed for students to gain hands-on experience working closely with our admissions team at Greenfield Health Systems (GHS).
T his role requires excellent sales and customer service skills, and is responsible for assisting in the placement of new and existing patients with GHS.
The student will coordinate with various referral sources, secure schedules, and ensure patients receive financial and medical clearances for lifesaving dialysis.
This is a customer-facing role requiring a detail-oriented individual capable of multi-tasking and managing cases while making and receiving multiple phone calls.
PRINCIPAL DUTIES AND RESPONSIBILITIES : *
- Assist in placing patients using excellent sales and customer service skills.
- Perform case management and serve as the primary liaison with referral sources, clinics, and discharge planners.
- Obtain chair times, financial clearance, and medical clearance to expedite the admissions process.
- Serve as the primary contact for initial patient referral calls and communications regarding patient admission to clinics (chronic and / or acute) and home programs.
- Coordinate the admissions process from the initial request to patient placement in the preferred clinic.
- Collect, complete, and communicate all intake data for admissions requests according to established standard operating procedures and regulatory requirements.
- Navigate various levels of clinic and management staff to resolve issues.
- Obtain necessary demographic, financial, and clinical information to complete the admissions process.
- Assist other teams during transient or busy periods, including working overtime when required .
- Maintain and update Careport with current data, pertinent information, and status of admissions referrals.
- Complete preliminary paperwork and standard admissions forms to ensure efficient processing of admissions.
- Promote the efficiency of the admissions process by answering phones and assisting callers in a professional and timely manner.
- Handle escalations and collaborate with supervisors / managers as necessary.
- Communicate with individual clinics as needed to provide prompt scheduling of patients and forward necessary documents and records to the appropriate receiving facilities.
- Collaborate with facility staff and various levels of management to effectively resolve issues impacting a patient’s admissions process.
- Perform other duties as assigned.
EDUCATION : *
High School Diploma required
EXPERIENCE AND REQUIRED SKILLS : *
- Excellent sales and customer service, communication, organizational, and interpersonal skills required .
- Detail-oriented with the ability to enter information accurately into a database system.
- Working knowledge of standard office applications email, word processing, spreadsheets, and presentations.
- Ability to adapt to change and support new policies and procedures positively.
