The Ohio State University Wexner Medical Center is committed to enhancing the quality of care our patients receive. System wide Addiction Services serves the patients of the Wexner Medical Center by facilitating patient care across our Emergency Department, Inpatient and Outpatient areasof the Wexner Medical Center.
The Patient Navigator assigned to this program supports patients through the health care system to coordinate clinical services, enhance patient engagement, and provide information and resources that reduce barriers to care.
Position Summary
Patient Navigation directs patients to different resources and information in the health care system. Reaches out to and follows up with patients upon referral to ensure patients are engaged in addiction treatment and receive timely delivery of medical and psychosocial services.
Provides education, other resources and community supports to enhance care delivery and patient engagement. Maintains communication with patients, families, other program staff, and health care providers to assist the patient through their treatment and recovery journey.
Position includes data entry and is involved with developing referral pathways and partnerships with other agencies.
Required Qualifications : A Bachelor’s degree or equivalent experience required. Experience in patient navigation or other related experience desired.
