About the Role : We are looking for a Nurse Practitioner to join our Clinical team. The Nurse Practitioner will conduct Health Risk Assessments for Medicare, Medicaid, and Commercial Marketplace members / patients of primary care physicians, medical groups, and the health plans we serve.

The Nurse Practitioner will perform comprehensive assessments that are compliant with the annual wellness visit in the patient's home.

Location : The position is based in Las Cruces, New Mexico, and surrounding areas, and will require the applicant to have reliable transportation to travel to the patient's home.

What We OfferIndependent ContractorFlexible schedule & part-time positions available1-hour session per visitCompensation : $100 per completed visitWhat You'll Do : Complete a detailed evaluation of the medical and social assessment of the beneficiary members.

Identify and refer members / patients for routine, urgent, and emergent case management.Make recommendations for member's follow-up with their primary care physician.

Not required to prescribe or alter the member's current treatment plans.Complete comprehensive assessment face-to-face in-home setting.

Who You Are : 2 years of hands-on experience as a Nurse PractitionerMust be registered with Medicare and MedicaidCurrent, unrestricted license in the state of assignmentMust have license / certification : APRN, Certified Nurse PractitionerKnowledge of the HCC model, HEDIS, and STAR quality measuresEffectively communicate with elderly and chronically ill membersProficient in the use of computer, tablet, and mobile technologyAbility to document within available EMR.
