Case Manager Temporary to Hire, Full-Time Onsite, Boothwyn, PA $23-$26
Are you a Human Services Professional eager to make a difference? Join this Delaware County non-profit as a Case Manager to assist Veterans transition from homelessness to housing stability.
This position will engage and initiate resolution services for Veterans to divert from homelessness, conducting coordinated assessments, intakes, and determining eligibility for Veteran households who are homeless or at imminent risk for homelessness, and ensure that immediate interventions and housing stability services are initiated for eligible Veteran households.
A Day in the Life of a Case Manager
- Conduct initial interviews with Veterans facing a housing crisis in an effort to divert from homelessness, through landlord mediation and problem-solving strategies.
- Conduct coordinated assessments for Veterans and their families facing a housing crisis and initiate a housing stability plan.
- Provide intake, assessment and critical need intervention for Veterans and their families in need of assistance.
- Verify and determine Veteran eligibility for service, including computing income eligibility, and assisting Veterans in obtaining all necessary eligibility documentation.
- Rapidly exit Veterans from homelessness through the provision of temporary financial assistance, case management services, linkages to natural and community resources, and by empowering Veterans using a strength-based and client-centered approach to services.
- Develop and utilize a broad network of Agency and Community resources to identify and channel Veterans to the best possible path and programming.
Requirements of a Case Manager
- Minimum requirement of a bachelor's degree in the Human Services field. A master's degree in the Human Services field is preferred.
- Requires an appropriate combination of education and experience that fulfills the knowledge and experience base required for this position, including training / education and work experience as it relates to Housing First and Progressive Engagement principles.
Experience in working with the homeless and with Veterans from a trauma-focused and strength-based approach is preferred.
- Must demonstrate outstanding organizational skills, and the ability to manage multiple priorities and concerns.
- Knowledge of the Delaware County Social Service infrastructure and the ability to maintain effective working relationships with service providers.
- Computer literacy including familiarity with Word and Excel, and willing and able to learn a CoC database.
- Excellent communication skills, both oral and written, combined with strong interpersonal skills and the ability to work well with people from different cultural and socio-economic background
