Congreso is hiring for both Attendance Case Managers and General Case Managers in the Community School Program.

Attendance Case Manager Position Summary

The Attendance Case Manager is responsible for improving attendance at their assigned Community School and helping students develop positive patterns of school engagement.

From within a student's school, the Attendance Case Manager uses client-centered methods to engage families, assesses needs, contributes to increasing confidence and motivation for student success, and provides education on school policies, resources, and connection to supportive services.

This position works collaboratively with school personnel, families, Truancy Regional Court, and other partners to support students.

The Attendance Case Manager is paired with students who have 3 or more unexcused absences for up to 90 days of service, and for students with 10 or more unexcused absences, the Attendance Case Manager works to resolve barriers by conducting business in the student's home and neighborhood and is present with them as they navigate through the judicial system, which may include Regional Truancy Court and Family Court appearances.

This position manages a small budget for student incentives to improve attendance and host community engagement events at the school.

This position reports to the Community School Program Manager . This is an ONSITE position, primarily conducted at the school site, and includes home visits within the community, and occasional court appearances with students and their families.

Attendance Case Manager Essential Functions

Case Management

  • Represents Congreso's mission, values, and commitment to diversity, equity, inclusion, and belonging and be an ambassador of our work.
  • Collaborates with Community School Coordinators to provide direct support to students that are truant and provides incentives to students to support regular school attendance.
  • Provides individual interventions to students struggling with attendance using short-term intervention of no more than thirty (30) days to remove barriers for students attending school regularly.
  • Tier 1 Services : Is paired with students who have 6 or more unexcused absences and have had a Student Attendance Improvement Conference (SAIC) for up to 90 days of service;

provides intervention to remove attendance barriers by engaging families, assess needs, increase confidence and motivation for success, and provide education, resources, and connection to services.

  • Tier 2 and 3 Services : Conducts outreach to the student's home and school within funder specified time frames, engages students and their families, completes assessments, provides relevant education and follow-up support to students, families, and schools, attends all Truancy-related court hearings alongside families, provides monthly support to the student and family through home and school visits, and supports the family until successfully discharged from the program or transferred to more intensive services.
  • Provides general case management services to students in need of social services, health navigation, and / or needs related to family stability, when the demand for those services is high.
  • Collaborates with the General Case Managers at the school site to equitably distribute both Attendance and General case management cases.
  • Adheres to documentation deadlines and documents cases in detail, which includes but is not limited to submission of written Court Report testimony for all hearings, case note entry into digital database systems within 5 days of the contact event, neat and tidy paper files for each case, etc.
  • Participates in weekly supervision and comes prepared with up-to-date case notes and summaries.
  • Promptly reports all conflicts among student families, and / or serious incidents involving student and families within 24 hours of the incident.
  • Provides services and support outside of working hours to accommodate a family's emergency needs, when necessary.

Attendance Team Coordination

  • Supports the school's Attendance Team to identify students in need of support, remove the barriers to attending school, and slow or stop the accrual of absences, ultimately reducing the number of referrals to Regional Truancy Court.
  • Develops a strategy for incentive funds allocated per school in collaboration with the Community School Coordinator and School Principals.
  • Manages requests for attendance incentives and provides monthly and ad-hoc reports.
  • Attends monthly Community School Partner Meetings and provides support to the meetings with provider agency resources, attendance events.
  • Participates in sub-committees related to attendance initiatives at the school.
  • Meets weekly with the Community School Coordinator to discuss events, incentive initiatives, school-specific successes or challenges, etc.

General Case Manager Position Summary

The General Case Manager collaborates with school personnel and families to support student success through holistic case management services.

These services include referrals to social services, health navigation, socio-economic supports, and meeting other needs to support family stability.

From within the student's school, the General Case Manager uses client-centered methods to engage and advocate for families, assesses needs, contributes to increasing confidence and motivation for student success, and provides education on school policies, resources, and connection to supportive services.

This position is responsible for conducting home visits, strength- and needs-based assessments, developing a service plan, and implementing problem-solving interventions, including coordinating warm handoffs to community programs and / or services.

General Case Manager Essential Functions

  • Provides general case management services to students in need of social services, health navigation, and / or needs related to family stability.
  • Assesses family needs, generates service plans, and collaborates with families to meet their goals and provides referrals to supportive services.
  • Provides school-based attendance support by removing barriers to attendance for students on the caseload.
  • For students involved in the Family Court or Regional Truancy Court systems, attends court and acts as an advocate, writes Court Reports and tracks progress towards goals until the case is discharged.
  • Safeguards the confidentiality of client families.
  • Conducts home visits for identified families.
  • Promptly reports all conflicts among client families, and / or serious incidents involving client and families within 24 hours of the incident.
  • Provides services and support outside of working hours to accommodate a family's emergency needs, when necessary.
  • Adheres to documentation deadlines and keep detailed, clear case notes. Case Managers are to submit written testimony in the form of a Court Report for all hearings.

Case Notes are to be entered in digital database systems within 5 days of contact event. All Case Managers will also maintain a neat and tidy paper file for each case.

Ability to navigate the internet and digital data systems required.

  • Prepares for and attends supervisory support meetings including one-on-one supervision, case reviews, team meetings, etc.
  • Keeps quality case files that pass all internal and funder file audits.
  • Completes assessments, home visits, and other required activities within the standard timeframe.
  • Ensures program quality assurance tools are used and updated according to deadlines.

  • Bilingual (Spanish / English) written and verbal skills a plus .

Supervisory Responsibilities :

This position has no supervisory responsibilities.

Minimum Experience, Education, and Qualifications

Minimum Education :

Bachelor's degree insocial work or related field required. Examples of related fields include : social services, sociology, psychology, theology, education, criminal justice, human services and public administration.

Minimum Experience :

1 year of experience in social work or a related field that includes case management, education, and child welfare experience working with children and youth, juvenile delinquency, and / or families.

Certification / License :

  • Mandated Reporter and Supporting Children and Families with Disabilities Certificates required and may be completed post-hire.
