JOB DESCRIPTION Maxim Healthcare Services is seeking a Nurse Clinical Supervisor who is responsible for coordinating the continuum ofcare activities for assigned patients andensuring optimum utilization of resources, service delivery and compliance with medical regime.
Schedule : Monday-Friday 8 : 30am-5pm Responsibilities :
- Performs and coordinatestheinitialassessmentsandongoingreassessmentsof the patient's status
- Documents patientcaseinformationwithin adatabasesystem
- Performs chartreview / audits monthlyor as needed
- Participates inmonthly caseconferences by providing information pertinentto patient’s needs / goals
- Partnerswiththe ProgramDirector indevelopment and review of thepatient’s individualizedcoordinationofcareplan
- Ensures thatthepatients’medicalneeds areaddressed;consultswith thepatients’ physiciansasneeded, coordinating plans oftreatment,and advocating for the patient whennecessary
- Promotes understandingofthe medicalfactors affecting thetargeted population
- Identifies and assists patient(s)in accessingentitlements, resources,information,and referrals for psychosocialneeds
- Participates inQuality Assuranceand Utilization Reviewactivities,asdirected
- Empowers patients in decision-making forcare planning
- Maintains accurate and timelypatientinformation, which is readily accessibleforreview and meets allrequirements;assists in datacollection for reporting / funding sources
- Fosters intra-facilityand inter-facility working relationships tohelpaccomplish goals ;acts asa liaison between primarycareproviders,specialist,and / orpatient
- Advocateson behalfofpatientregarding accessibility of services
- Follows State / Country mandated guidelines for thenursecase managementprograms
- Participates inoutreach activitiestothe entire targetpopulation,as directed
- Recommendsprogram / service changes to meetgaps in serviceinthecommunity
- Performsother duties asassigned / necessary
Qualifications :
- CurrentRN license
- Atleastoneyearof experiencepreferred
- CurrentBLScard percontractrequirements
- CurrentTBorChestx-ray (if applicable)
- Ability to effectivelyelicit / provide information toandfromappropriateindividuals(including,but notlimited to,supervisors,co-workers,clients) viastrong communication skills;
proficiencyin theEnglish language mayberequired
- Computer proficiencyrequired
- Mustbe atleast18yearsof age
