Responsible for coordination of care at designated site, inclusive of care path facilitation, data collection and application of criteria to identify and manage general and high-risk patients.
Facilitates interventions, staff assignments and coordination issues utilizing system wide criteria as tools. Must possess working knowledge of managed care, inpatient, outpatient, and the home health continuum, as well as utilization management and case management.
Incumbent must maintain their knowledge of leading practices in clinical care and payor requirements. Incumbent must maintain a working knowledge of the concepts associated with Performance Improvement.
