CTI Case Manager

Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.

Main Function :

With minimal supervision, as part of the CTI (Critical Time Intervention) team and in keeping with Agency policies and procedures and professional standards and applicable regulations, use own experience and judgment to carry out the mission of the Critical Time Intervention model (CTI) for youth boarding in hospitals and ERs.

Responsibility includes provision of CTI services for assigned youth, developing relationships with assigned youth / family, developing relationships with hospitals and service providers, following the fidelity of the CTI program, and completing assessments, progress notes, and reports as required.

Key Responsibilities :

  • Provision of CTI (Critical Time Intervention) Services
  • Meet with youth and family referred from the hospital for CTI services.
  • Complete intake assessment to determine needs and goals.
  • Provide intensive case management for the assigned youth / family for a 9-month period.
  • Assist the youth / family in determining additional services needed to achieve and maintain stability.
  • Develop a schedule of visits with the youth / family based on CTI requirements and the youth / family’s needs.
  • Observe progress with supports / referrals and make changes as needed.
  • Maintain Appropriate Records and Prepare Necessary Reports
  • Complete an intake assessment and goal plan for each youth / family assigned.
  • Ensure that all required documentation is completed and maintained per agency / department standards.
  • Document contacts / referrals made for each youth assigned.
  • Ensure incident reports or compliance incidents are recorded and reported per agency procedures.
  • Community and Teamwork
  • Establish and maintain rapport with hospitals, care managers, providers, etc., to assist with referrals and obtaining services for the youth / family.
  • Participate in all scheduled meetings with the youth / family and team.
  • Meet with CTTP Director as scheduled to discuss CTI implementation, timelines, and progress of each youth.
  • Schedule and participate in a celebration meeting for youth / family at the end of the CTI process.
  • Follow Agency Standards and Procedures
  • Understand and follow the CTI process.
  • Provide person-centered quality services.
  • Other Duties
  • Perform other related duties as assigned.

Education & Experience :

A Bachelor’s degree in a Human Service field is required. One year of experience working with people with developmental disabilities or a mental health diagnosis or case management experience working with youth and families is preferred.

The position requires good verbal and written communication skills. Candidates must be able to demonstrate teamwork and professionalism.

Basic word processing and e-mail skills are required.

Conditions of Employment :

Candidates must have a valid NYS Driver's License in accordance with the Agency Vehicle and Driving policy and have transportation necessary to meet program needs.

The role requires on-call availability for bona fide emergencies and after-hours program service needs. Schedule flexibility is required, including occasional early morning, evening, or weekend meetings / functions.

Candidates must be willing to meet with youth / family in hospital, home, and community settings. Successful participation in required database checks through the State Central Register of Child Abuse and Maltreatment, HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General, and Specially Designated Nationals is mandatory.
