Overview
We're looking for a Licensed Practical Nurse - Continuous Care to join our hospice team.
This position will report directly to the Executive Director / Administrator or Director of Clinical Services, but work under the direction of the Registered Nurse to provide continuous care visits to patients and families in a facility / unit or home setting.
2nd shift, 4pm-Midnight
bedside, "sit care"
participate in On Call rotation, if not on a Continuous Care assignment
The Licensed Practical Nurse Continuous Care will actively monitor patients and report changes in their condition.
Achieve palliation and management of acute medical symptoms during brief periods of crisis
Under the direction of an RN, provide patient care services to hospice patients and supportive care to the patient's family with a reasonable case load
Evaluate patient / family needs and assist in the preparation and execution of the plan of care for each patient
Provide direct patient care, including administering medications and monitoring vital signs
Provide education and support to patients and their families regarding their healthcare needs
Report findings or changes in the patient's condition to the Registered Nurse, interdisciplinary team and referring physician and document all reports
About You
Graduate of an approved school of Practical Nursing
A minimum of two (2) years' experience as an LPN / LVN, either in long-term care, hospice, or home health.
Three to five years' experience preferred
Compassionate, kind and empathetic to our patients and their families
Ability to assess and respond to the needs of patients, families and caregivers
Certified in CPR
