AVH+ is a locally owned company with owners actively participating in the day-to-day operations of the agency performing as part of the team.
We are fully staffed and looking to expand our team in order to support and anticipate our growing patient census's needs.
Position Summary :
We are seeking an experienced, compassionate, and fluent Spanish-speaking RN Case Manager to join our team. In this role, you will provide quality hospice care, coordinate patient care plans, and serve as a trusted advocate for patients and their families.
This role requires a professional with a strong commitment to compliance and a compassionate approach to end-of-life care.
Responsibilities :
- Provide direct patient care, assessment, and follow-up, focusing on symptom management and comfort.
- Develop, implement, and regularly update individualized patient care plans.
- Educates patients / caregivers regarding hospice philosophy, disease progression, physical components of care, symptom management and end-of-life care options, stages of dying, and other aspects of the POC as necessary.
- Maintain accurate patient records and documentation in accordance with AVH+ Hospice policies and procedures.
- Collaborate with interdisciplinary team members to ensure continuity of care.
- Serve as a liaison between patients, families, and healthcare providers, providing support and information in both English and Spanish.
- Document patient progress and communicate changes to the healthcare team and family members.
- Maintain compliance with hospice policies, Medicare, and state regulations.
- Regularly attends and participates in IDT meetings to ensure that the POC reflects current patient care needs.
- Promotes and maintains respect for rights, privacy, and confidentiality of others.
- Respects patients and staff confidentiality always.
- Attends staff meetings, mandatory in-services, and other meetings as appropriate or upon request.
- Assumes responsibility for own personal and professional growth including state licensing / certification requirements.
- Understands and exemplifies hospice philosophy, values, beliefs and team concepts in all job-related roles.
Qualifications :
- Active RN license in California.
- Hospice and / or palliative care experience preferred.
- Fluency in Spanish (written and spoken) required.
- Strong understanding of hospice philosophy and end-of-life care.
- Excellent communication skills and a compassionate bedside manner.
- Ability to work independently and as part of a diverse team.
