PEDIATRIC PALLIATIVE SPIRITUAL CARE COUNSELOR

Baystate Health is hiring a Pediatric Palliative Care Spiritual Counselor to join

the home health team. The Spiritual Care Counselor plays an integral part to the Pediatric Palliative Home Care program, which is made up of a dedicated, compassionate team of experienced caregivers that work closely with each family to ease the symptoms, pain, and stress of serious illnesses.

Palliative Care teams work with the families of a child facing an illness that limits quality of life or significantly lowers the normal life expectancy to determine the best way forward.

The goal is to improve quality of life for the child and family from the comfort of their own home.

Summary :

The Pediatric Palliative Care (PPC) Spiritual Care Counselor performs services requiring spiritual assessment and support consistent with the beliefs and values of the patients and families.

The PPC Spiritual Care Counselor utilizes culturally and linguistically appropriate methods that honor patients and families.

The PPC Spiritual Care Counselor serves an important role as a facilitator between community spiritual, religious, and related organizations and patients and families.

Job Responsibilities :

1) Provide spiritual counseling care and support in accordance with the patients plan of care and promote spiritual well-being and thriving based on individualized goals and needs with a foundation of respect for all intellectual, cultural, spiritual, and religious traditions and beliefs.

2) Develop, implement, and update as needed an individualized plan of care according to the goals and wishes of patients and families and as requested by the patients and families

3) Document spiritual counseling in the patient's medical record in accordance with Baystate Home Health and Hospice procedures and policies.

4) Work collaboratively as a member of the interdisciplinary team to ensure patient's and families needs are met, continually evaluating patient response to care.

5) Attend and actively participate in interdisciplinary group (IDG) meetings, staff meetings, family events, and other meetings as requested and expected for the position.

6) Facilitate linkage to community faith-based leaders as requested by patients and families.

7) Maintain quality, competency, and productivity standards and participate in the measurement of these standards.

8) Develop and provide educational in-service programs for staff regarding spiritual aspects of care as requested by the manager.

9) Provide emotional and spiritual support to staff and volunteers regarding stress associated with providing pediatric palliative care through planned and spontaneous activities based upon need and under the direction of the manager.

10) Participate in the planning and program for memorial services for Baystate pediatric palliative care families.

11) Provide bereavement services and support to families as requested.

Required Work Experience :

A minimum of three years of experience as a chaplain, spiritual care counselor, pastoral counselor, or other religious / spiritual counselor, preferably in a health care setting.

Preferred Work Experience :

  • Experience working with individuals in an interfaith environment and respectful of all religious and spiritual beliefs.
  • Experience working with children, teens, and their family members.
  • Knowledge of caring for patients and families in the home setting.
  • Knowledge and experience providing bereavement support.
  • Spanish language skills preferred

Skills and Competencies :

Knowledge of computer information systems and the ability to work independently, Excellent verbal and written communication skills, Demonstrates competence in organizing and presenting educational programs, Good listener with excellent inter-personal skills
