Griswold Home Care is looking for a fantastic individual to provide field support to our clients and caregivers throughout Maricopa County.
The Lead Caregiver will work with the Case Management team by filling in on new cases and priority shifts, helping to eliminate scheduling gaps, and will also assist with the onboarding of new clients by refining care plans and care instructions.
The ideal candidate is flexible, dynamic, and has excellent problem-solving abilities to help give our clients the 5-star service that we promise!
Minimum Qualifications & Attributes
- High school diploma or GED and ability to do basic math.
- Language and literacy skills are adequate to draft / modify client records and communicate the needs and requests of clients.
- 5+ years of experience in caregiving.
- Must be willing to commute and have reliable transportation, a valid driver's license, and insurance.
- 3 professional references and cleared fitness to work.
- Ability to pass a thorough criminal and background record check.
- CPR & first aid certification and TB clearance (updated annually).
Essential Duties and Responsibilities
- Assist the Case Management team with filling in on scheduling gaps.
- Conducting client visits, assessments, re-assessments, and case launch visits to ensure client care plans are accurate.
- Provide exceptional customer service and conduct quality assurance visits on cases throughout the valley, as needed.
- Give encouraging feedback and coaching to caregivers where needed.
- Collaborate with medical professionals and company referral sources.
- Notify relevant parties of client changes, family situation, and case needs.
- Work with others on the admin team, problem-solve, and exhibit good judgment.
- Provide administrative support and assistance in the office as needed.
- Other tasks and caregiver duties as assigned.
