JOB OVERVIEW :

Joseph J. Peters Institute, a nationally recognized provider serving child, adolescent, and adult survivors of trauma as well as perpetrators of sexual abuse, seeks a Case Manager in the Safety and Responsibility Program.

This individual will support outpatient activity, facilitate scheduling intake appointments, complete chart audits, and collect, evaluate, and disseminate performance data.

The Case Manager reports to the Clinical Administrative Coordinator.

RESPONSIBILITIES :

  • Communicate with stakeholders regarding referral details and collection of important participant information.
  • Evaluate and determine payer information for incoming evaluations.
  • Schedule evaluation / intake appointments with participants or appropriate guardians.
  • Complete intake packets with all incoming participants.
  • Under general supervision, coordinate case management activity in the Safety and Responsibility Program.
  • Assist supervisor in providing administrative support to clinical staff.
  • Facilitate tracking of all participants being admitted / maintained in SRP.
  • Assist in developing and implementing new, innovative program strategies.
  • Collect, evaluate, and disseminate performance data relative to specified quality improvement programs within the outpatient clinic.
  • Conduct quality review audits as appropriate to the agency.
  • Generate various reports regarding performance against quality indicators, as appropriate to the individual position.
  • Participate in the planning, development, coordination, and presentation of specific training and educational programs as appropriate to the quality assurance needs of the agency.
  • Assist other facility departments as needed.
  • Assist the Youth Outpatient program as needed.
  • Assist other facility departments as needed.

SKILLS :

  • Ability to meet timelines
  • Ability to communicate in writing and orally
  • Good analytical and troubleshooting skills
  • Knowledge of resources available to staff and clients that enhance, coordinate, and complement the services provided at JJPI
  • Preferred : Working knowledge of PowerPoint, Excel, and SPSS
  • Preferred : Bilingual in Spanish and English

EDUCATION : Bachelor’s degree in a field related to clinical work

EXPERIENCE :

  • 2-3 years of experience in the field
  • Experience with human services systems

SALARY GRADE : 15
