Overview
Hospice of Chattanooga is looking for a self-motivated and passionate Hospice RN Liaison to join our Intake 365 Team. Our RN Liaisons work closely with our intake team and hospital partners to admit patients into our hospital in-patient programs efficiently, and provides the patient and family with the appropriate resources needed at the end of life.
Our RN Liaisons are responsible for coordinating appointments and explaining hospice services / philosophy of care to patients and families referred to Hospice of Chattanooga, along with the completion of the required documents.
Registered Nurse Liaisons use their clinical knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services.
Our RN Clinical Liaisons are hospice experts and serve as a resource to their referral sources to ensure extraordinary patient and customer service.

Responsibilities
- Conducts initial meeting with the patient and family giving an overview of hospice service and philosophy and obtains consents for care.
- Works independently to complete admission paperwork timely, understanding a reasonable goal with set expectations and demonstrating flexibility with assignments and all situations.
- Determines primary decision makers and pro-actively builds positive, solid working relationships with members of the medical community and partners.
- Promotes and positions Hospice's patients care service through regular visits in a manner that illustrates and reflects the Hospice's quality of care and unique capabilities.
- Works to build the brand of the hospice as the preferred hospice provider in our service area to achieve and exceed agency goals for growth and census management.
- May help Hospice in determining patient eligibility, based on their hospice knowledge and training.
Qualifications
- Bachelor's degree in nursing or health care preferred.
- Holds current unencumbered license as a registered nurse.
- Minimum of three years customer service experience (face-to-face) including sales. Healthcare customer calling a plus or working experience in the medical community
- One (1) to two (2) years’ experience in hospice / palliative marketing strongly preferred.
- Skilled at establishing / maintaining working relationships with key staff, marketing contracts, and clinical staff to ensure thorough understanding of Hospice services.
- Proven ability to generate and maintain relationships with new and existing accounts including physicians, healthcare professionals, and community agencies.
- CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption.
This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.
