The health care practitioner (HCP) is responsible for prescribing medication to alleviate withdrawal symptoms and cravings in individuals with substance use disorders and supporting their journey to recovery.
Help patients engage effectively in counseling and therapy, ultimately improving treatment outcomes and reducing the risk of relapse.
They provide health assessment, early detection of illness, health promotion, disease prevention and management. The HCP works to achieve State and Federal standards in both quality and productivity and may develop / coordinate special program related to area of expertise.

Practice opportunity details include :
- Candidate type : MD, DO, NP, PA
- 100% outpatient; Monday-Friday schedule (20-40 hours)
- Call is approximately 1 : 8 telephone only (varies with the number of providers)
- Team-based care : Medical team includes a team of physicians, NPs and PAs
- Integrated care with Dental, Behavioral Health and Psychiatry
- Paid malpractice with 100% tail insurance covered through the Federal Torts Claims Act (FTCA)

ESSENTIAL DUTIES / RESPONSIBILITIES :
- Offer a holistic approach to treating substance use disorders by combining medication with counseling and behavioral therapies, thereby promoting long-term recovery and improving the quality of life for individuals affected by addiction.
- Performs comprehensive health assessment through patient / family history taking, physical examination and assessment of response to actual or potential health problems.
- Orders and interprets tests and diagnostic studies as indicated and within patient care guidelines and standards of care.
- Establishes diagnosis based on assessment and appropriate diagnostic tests and procedures.
- Implements treatment plans within established medical protocols and determines plan for reassessment.
- Coordinates follow up contacts with patients as needed regarding abnormal test results, need for additional treatment, referral to specialist(s), etc.
- Determines individual patient and family educational and informational needs and provides / coordinates knowledge acquisition.
- Includes health promotion, prevention, and maintenance in health instruction relevant to the patient.
- Records all assessments, observations, treatment plans, interventions, and evaluation in the medical record.
- Completes patient encounters and submits data per policy.
- Addresses clinical tasking and provider approval queue per policy.
- Delegates to clinical staff assessments, treatments, or tasks appropriate to licensure, competency, and knowledge.
- Establishes and maintains appropriate clinical privileges.
- Provides care at all-site locations as assigned.
- Maintains productivity.
- Collaborates with the health team and other community agencies in providing comprehensive care.
- Provides appropriate internal and external referrals to dental, psychiatry, behavioral health, medication programs, case management, outreach and enrollment and specialties to provide the patient with the best interactive care to address biological, psychological, and environmental factor needs.
- Prescribes and regulates medications within the scope of practice.
- When indicated, will maintain a collaborative agreement.
- Maintains and enhances a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure.
- Accesses resources for indigent, uninsured, disabled, or medical underinsured patients and families from federal, state, or local programs.
- Regularly attends and participates in provider staff meetings.
- Participates in Continuous Quality Improvement committees as assigned by the Chief Medical Officer.
- Participate in the on-call rotation per policy.
- Participates in peer review per policy.
- Participates in prescription drug monitoring program.
- Performs other duties as required within the limits of licensure.
EDUCATION / EXPERIENCE REQUIREMENTS :
- Doctor of Medicine, Doctor of Osteopathic Medicine, or certification and completion of Nurse Practitioner or Physician Assistant Program.
- Current license to practice in the State of Illinois.
- Comprehensive understanding of addiction medicine, pharmacology of MAT medications, and clinical management of substance use disorders.
- Training and certification in addiction medicine or psychiatry.
- Extensive knowledge of physical assessment, diagnosis, pathophysiology, pharmacology, and management of acute and chronic illness.
- Able to work with patients from a variety of socioeconomic and cultural backgrounds.
