Job Summary

The Community Health Worker is responsible for helping patients and their families navigate and access community resources, providing disease care and / or prevention education, providing social support and informal counseling, and encouraging patient engagement in reaching goals and adopting healthy behaviors.

Job Specific Responsibilities

Daily assignments may include but are not limited to :

  • Provide community service outreach; connecting patients to resources and services to address their social needs
  • Support providers, care coordinators, and others through an integrated approach to care management and community outreach
  • Educate clients on disease prevention; Make referrals and assist with coordinating services
  • Assist clients in self-management of chronic illnesses and medication adherence
  • Organize and / or facilitate support groups
  • Provide accuracy in tracking and documenting efforts, follow-ups, referrals, etc..
  • All other related assigned duties

Education and Experience

  • High School Diploma or GED
  • 1 years of experience in serving as a community health advocate
  • Certification as a Community Health Worker in Texas or formal training
  • A resident of Lubbock County for at least 1 year

Required Licensures / Certifications / Registrations

Valid Community Health Certification within 180 days of hire into the role

Skills and Abilities

  • Excellent customer service skills and the ability to resolve complex customer service issues and exercise conflict management
  • Ability to read, write, and speak English in a clear, concise, professional manner; Includes active listening skills and understanding medical and social needs of the patient population
  • Proficient computer skills; Knowledge of various software(s) and the ability to learn new software that may be needed for tracking or operating community relations efficiently
  • Demonstrated ability to work with diverse and cultural differences amongst people
  • Knowledge of HIPPA Guidelines

Interaction with Other Departments and Other Relationships

This position will work closely with community benefit providers, social services team, care coordinators, providers, and other team members to ensure quality outcomes for patients

Physical Capabilities

Work is of medium demand; walking, sitting and standing most of the time while on duty. Occasional lifting of patients with assistance is required.

Adequate hand / eye coordination and fine motor skills required. Talking and hearing is essential in instructing and teaching others.

Near visual acuity required to work with charts and records and to observe patients.

Environmental Working Conditions

Works in a well-lighted, heated and ventilated building. Required to do home visits, therefore, dependable transportation is required.

Hours may vary to accommodate needs of the patients. Exposure to blood borne pathogens is a risk.
