This is a new position that will be based out of one of our facilities in Springdale, Bentonville, or Lowell. The Triage LPN will field calls, assess needs, reference medical information, route messages etc.

As Northwest Arkansas' premiere hospice provider, we operate two beautiful inpatient hospice care facilities and offer outpatient hospice services for Benton, Washington, Madison, and Carroll counties.

General Description : The LPN is an integral part of the hospice interdisciplinary care team. This position triages patient calls, attends deaths, and makes home visits as needed.
As part of the regular work schedule, may be required to attend weekly team meetings and other staff meetings as requested.
May be required to take referrals, act as Nurse Liaison and admit patients into the outpatient hospice program at times.
This position is eligible for job share.
Requirements : Current unrestricted AR LPN license, BLS certification, strong assessment skills with critical thinking and high level of problem solving within a healthcare facility.
Strong documentation and communication with the ability to stay calm and cope with human suffering / emergencies. Prefer hospice experience, home health, or acute care nursing experience with computer charting experience.
