JOB SUMMARY :
The Registered Nurse plans, organizes and coordinates home health care and services for the patient and family / caregiver.
YOUR IMPACT AND RESPONSIBILITIES :
- Performs initial and ongoing assessment of the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
- Coordinates and implements the individualized plan of care recommending revisions to the plan as necessary.
- Consults and educates the patient and family / caregiver regarding health process, self-care techniques, and goals.
- Initiates appropriate preventative and rehabilitative nursing procedures.
- Prepares clinical and progress notes that demonstrate progress toward established goal(s).
- Coordinates all patient and family / caregiver services and prioritization of needs with members of the interdisciplinary group.
- Utilizes case management approach and referring to other services as needed.
- Informs the physician and other personnel of changes in the patient’s needs and outcomes of interventions.
- Determines scope and frequency of services needed based on acuity and patient and family / caregiver needs.
- Supervises LPNs and paraprofessionals providing services to the patient according to regulatory guidelines.
- Participates in in-service programs.
- Provides specialized training to other staff, family members and informal caregivers to insure exceptional care.
- Evaluates the patient and family / caregiver response to care.
- Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
- Evaluates own needs for support using identified system(s).
- Applies specific criteria for admission and re-certification to home health care to establish appropriate levels of care and the patient’s eligibility.
QUALIFICATIONS FOR SUCCESS :
- Registered Nursing degree (RN) from an approved school of nursing as determined by the Board of Nursing, required.
- Licensed to practice registered nursing in the state by the Board of Nursing, required.
- Current CPR certification.
- Computer savvy, able to easily navigate basic computer tasks and computer medical record.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills per competency checklist.
- Understands and supports the home health philosophy of care.
