Public Service Loan Forgiveness (PSLF) qualifying agency Job Summary The Clinical Co-Responder w ill work solely with adults (18+) experiencing homelessness with a co-occurring mental health and / or substance use disorder who have been identified as high-utilizers of shelter and emergency services in South Salt Lake.

The Clinical Co-responder will work closely with the community of South Salt Lake to achieve these outcomes : (1) reduce length of stay in shelter and / or period of homelessness;

2) reduce number of police interactions; (3) increase number of individuals diverted out of the court system; and (4) increase number of crisis calls diverted to case management vs first responder calls.

  • To produce those outcomes, the Clinical Co-Responder will follow these objectives : (1) implement the Critical Time Intervention (CTI) model for identified high-utilizers;
  • 2) maintain a partnership between South Salt Lake Police Department (SSLPD) and The Road Home (TRH); (3) connect clients to community resources for long-term care and stability;

28 / hour with $2 increase upon full licensure (LCSW or equivalent) FLSA Status Exempt JOB DUTIES Managing a Case Load Engage with and provide comprehensive behavioral health services to clients, empowering individuals using a strengths-based approach to develop problem-solving skills, create goals, and communicate effectively.

Build rapport, establish goals, and assist our diverse population of guests on your set caseload through frequent face-to-face interactions.

Engage closely with law enforcement to provide triage, crisis intervention, assess needs, provide resources, and connect with treatment for clients on caseload.

Respond to, assess and follow up on referrals from law enforcement, partnering agencies, and community members relating to clients.

Track court cases, warrants, and court appearances to assist clients with navigating the law enforcement system and resolving legal barriers.

Engage in crisis intervention and de-escalation as necessary. Focus on and implement RSS program and CTI model for clients, if / when applicable.

Assess clients' needs through mental health evaluation and assist or link clients with basic needs, as well as legal services, vocational services, health care, substance use treatment, and psychiatric services, including inpatient services.

Provide transportation to clients to and from court, meetings with attorneys, mental health and substance use appointments, physical health appointments, and other needs to help eliminate barriers to stability.

Keep and maintain accurate records, documentation, and statistical data as required using HMIS as main database and participate in special projects as needed.

Resource Collaboration Recruit, hire, train and supervise case management level positions. Provide weekly one-on-one supervision to direct reports.

Collaborate with internal staff, community partners, and Homeless Resource Officers (HROs) within SSLPD to facilitate communication, coordinate care, provide updates on progress and plans, and assist with needs of clients.

Collaborate with housing-focused case managers to engage in housing conversations with clients and assist them with transitioning out of shelter, as well as to support individuals on caseload with wrap-around services.

Refer and connect clients to mental health and substance use treatment resources and community partners for long-term care.

Attend weekly and monthly meetings with internal staff members, SSL officials, SSLPD, shelter security, and SSL Fire and EMS to identify high needs and concerns in the shelter and community in order to foster a healthy collaborative effort to best serve clients.

Meet with the court administrators and the judge, as well as prosecutors, to explain the resources available to our program and how we can most effectively work with them to accomplish the program goals.

Attend community meetings held monthly with city residents and businesses to address issues, concerns, and complaints. Advocate on behalf of clients regarding available resources in the community and systemic barriers impacting stability.

Maintain training, education, and best practices by attending trainings, conferences, and other opportunities for continuing education.

Demonstrated awareness of and sensitivity to diverse populations. Ability to contribute to the agency’s commitment to enhancing awareness and appreciation of diverse ethnic and cultural heritages.

Excellent writing and documentation skills. Willing to be a proactive member of a team. Ability to work from a Trauma-Informed Care framework.

Familiarity with the Housing First philosophy. Familiarity with Recovery Support Services (RSS) Familiarity with Critical Time Intervention (CTI) Familiarity with the Vulnerability Index Service Prioritization Decision Assistance Tool (VI-SPDAT) Ability to work with community partners.

Self-starter and self-directed. Ability to accept direction from Supervisor. Excellent organizational skills and strong computer skills.

Must pass a pre-employment background check and drug screen. Education and Experience Master's Degree graduate in a mental health discipline from an accredited university.

L.C.S.W. or equivalent preferred but will provide supervision for a C.S.W. working on licensure. Work or volunteer experience preferred Experience working with diverse populations Supervisory experience preferred Physical and Equipment Requirements Ability to lift 25 pounds.
