Description
Job Title : Registered Nurse (Home Health) - Taos, NM Full Time Perm
Shifts : Monday - Friday, 8 : 00 AM to 5 : 00 PM, with flexibility based on patient needs.
Be available for on-call duties on weekends, with on-call pay of $75 per day
patient ratio of 4-6 per day
Description :
We are hiring experienced Registered Nurses for Home Health and Hospice roles. Candidates should have 1-2 years of relevant experience, proficient in Kinsser / WellSky.
Responsibilities :
Conduct comprehensive assessments of patients in their homes, ensuring individualized care plans are developed and implemented.
Administer skilled nursing care to patients requiring various medical treatments and interventions.
Collaborate with interdisciplinary teams to coordinate and manage patient care effectively.
Utilize Kinsser / WellSky for documentation and communication to ensure accurate and timely record-keeping.
Provide support to patients ranging from middle age to geriatric, adapting care plans to meet their unique needs.
Maintain a ensuring personalized attention and quality care delivery.
Respond promptly to changes in patient conditions and modify care plans accordingly.
Collaborate with other healthcare professionals to ensure a holistic approach to patient care
Qualifications :
RN license and CPR certification
1-2 years of Home Health and Hospice experience
Proficiency in Kinsser / WellSky preferred
Strong communication and interpersonal skills
