Open Doors Transition Specialist (1.0 FTE)
Job Description
Purpose : Provide coordination and support to consumers seeking to leave nursing facility care and transition into community living utilizing home and community based services.
Reports to : Open Doors Regional Lead
Responsibilities :
- The Open Doors Program provides transition services to Albany, Columbia, Greene, Rensselaer, Schenectady and Schoharie Counties.
- Develop positive working relationships with nursing facility staff and program stakeholders.
- Conduct intake process for assigned consumer referrals.
- Ensure each consumer receives quality counseling, resources and supports throughout their transition process.
- Provide information about relevant services and refer consumers to programs when appropriate.
- Collaborate with the Program Lead to determine barriers to transition and identify strategies to meet the needs of each consumer.
- Maintain all physical and electronic records in files, and databases.
- Expand knowledge of home and community based services through participation in trainings and in-services.
- Perform other program-related duties as assigned
Qualifications :
- Minimum of a bachelor's degree in Human Services or related field, and two years of relevant experience
- Must be able to provide own transportation within six county region
- Excellent customer service and communication skills.
- Proficiency with Microsoft Windows and Office.
- Organized and self-motivated.
- Personal experience with disability is preferred.
- Understanding and acceptance of Independent Living Philosophy or willingness to learn about it.
