Job Title : Lead Clinical Case Manager

Status : Exempt, Full time

Job Summary :

This is an exempt position reporting to the Site Director. The Lead Clinical Case Manager is primarily responsible for the day-to-day activities, safety of the clients, all interventions, and case management services within our Veteran suicide prevention program.

Additionally, the Lead Clinical Case Manager provides leadership and supervision to program staff, i.e. Clinical Case Managers and Peer Support Specialists.

Job Duties :

  • Responds promptly to referrals from community partners, self-referrals, and other stakeholder referrals as necessary.
  • Acts as a direct supervisor to the program staff, ensuring all aspects of the grant are implemented and addressed.
  • Provides guidance, mentorship, and training to program staff.
  • Follows all Nation's Finest internal personnel policies, ensuring equitable treatment of all staff, and ensuring satisfactory performance levels are maintained within the program.
  • Responsible for approving staff vacation requests, ensuring adequate staffing support when team members are away from the office, and responsible for maintaining a professional, supportive, engaging, positive, and transparent work environment for program team members.
  • Responsible for hiring program staff, conducting performance appraisals, and following internal disciplinary policies as appropriate.
  • Oversees day-to-day supervisory tasks including timesheet approval, staff needs, time off requests and scheduling.
  • Maintains current knowledge of suicide intervention and prevention best practices through ongoing clinical education.
  • Conducts clinical assessments and completes client intakes. Ensures program staff are trained appropriately and conduct all assessments according to grant expectations.
  • In collaboration with the program team, creates community education materials, handouts, and curriculum for presentation to community partners and interested community members.
  • Provides emergency clinical intervention as necessary for Veterans experiencing crisis. Documents clear, concise, timely notes in client case files.
  • Maintains strong relationships with Department of Veterans Affairs staff members to ensure successful integration of Nation's Finest and VA suicide prevention programs.
  • Promptly and accurately reports any issues or concerns regarding clients to direct supervisor or designee. Maintains strictest confidentiality with regard to protecting client's privacy and record keeping.
  • Reviews the progress notes from the other providers and Clinical Case Managers to accurately determine the strengths and limitations of each Veteran being referred for services.
  • Educates Veterans, and their families of the options available to them and collaborates with the veteran and family on the preferred option(s).
  • Provide case management services to program clients as required by the grant funder. Makes timely warm referrals to community partners best positioned to help each individual Veteran.
  • Assesses, plans, implements, coordinates, monitors and evaluates the options and services required to meet the needs of Veterans at high risk for suicide or other self-harm.
  • Conducts weekly in-person outreach within the assigned region / community.
  • Perform other duties and tasks as assigned.

Required Job Skills :

  • Ability to work independently, with minimal supervision while maintaining an effective working relationship with other staff members.
  • High comfort level with public speaking and community education initiatives.
  • Ability to work within defined parameters of a grant-funded program, understanding that funder requirements are a priority.
  • Ability to maintain calm demeanor in stressful situations.
  • Ability to work collaboratively with a team.
  • Ability and willingness to work in an in-person office setting with a minimum of three days per week of in office work with the remaining day being community outreach
  • Comfort in learning new software systems, including cloud-based file sharing systems, electronic health records, and other similar systems.
  • Be detail oriented, able to set priorities, meet deadlines, and work in high-stress environments.
  • Able to set and maintain professional boundaries at all times with clients adhering to agency policy and procedures.
  • Excellent inter-personal communication skills with the ability to interact with participants of varying ethnic, social or economic backgrounds.

Qualifications :

  • Master's degree in behavioral health field is required. Licensure to practice psychotherapy in the state of California (LCSW, MFT, LCP) required.
  • At least five years' experience in a social services setting.
  • At least two years of supervisory experience within a similar job setting and level of responsibility
  • Experience in the provision of clinical intervention for individuals in crisis.
  • Knowledge of best practices in suicide prevention and intervention is highly desirable with a minimum of two years of direct experience providing suicide prevention services.
  • Working knowledge of the following systems : MS Outlook, MS Excel, MS Word, MS SharePoint (or other cloud-based file sharing system,) MS Teams (or other video conferencing system,) Electronic Health Record (or EMR.)
  • Reliable transportation, clean DMV, and vehicle insurance is required. Background checks are conducted on any candidate who receives a job offer.

Full-time positions eligible for comprehensive benefits package. Both full and part-time employees are eligible for generous paid time off and retirement contributions.

Nation's Finest offers a four-day workweek for full-time day staff. A casual, friendly environment and ample training opportunities are available.

$77,000 - $83,000 per year to start -DOE and regional placement. Eligible for annual COLA based upon funding availability.
