Description
Seeking full time Registered Nurse / LPN for evening or overnight shifts in a team environment providing care to adults and / or teens at CHD's Crisis Stabilization;
this is part of a single location providing crisis, outpatient behavioral health and brief stabilization care. The right candidate will have a sense of humor, be self-motivated and a team player with a harm-reduction orientation to care.
Shift differential and Sign-On Bonus available.
Essential duties for these roles may include :
- Collaborating with nurses, physicians, advance practice nurses, psychologists, social workers, or other professionals
- Collaborating with guardians, relatives, and significant others of people served when clinically indicated
- Documenting assessments and treatments in the electronic health record
- Monitoring vital signs
- Assessment of signs of substance withdrawal and completion of standardized assessment tools such as the Clinical Opioid Withdrawal Scale (COWS, CIWA, CIWAb)
- Fills physician orders
- Administers medications
- Engages in a medication reconciliation process
- Coordinates medical care
- Contributes to comprehensive assessments
- Brief crisis counseling
- Individualized crisis prevention planning
- Providing psychoeducation
- Assisting with discharge planning and care coordination.
